Our patients are required to complete several forms which will be provided to you upon arrival at your first appointment. For your convenience, we have also included the forms here for you to print, complete and bring to your first appointment, if you prefer.

Dr. Ararat has treated many types of client needs and specializes in the compassionate care of patients who have lost or damaged teeth and bone structure from cancer, trauma or other health concerns. These cases sometimes require maxillofacial jaw reconstruction along with prosthetic services to regain speech, comfort, the ability to eat, and most importantly, confidence. It is a privilege to meet new patients, learn about their individual stories and needs, and provide them with lasting treatments that help them regain a renewed sense of self.

It is important to follow instructions after you have oral surgery to ensure proper healing and to avoid complications. The information found on our Post-Op Instructions page are guidelines. After your surgery the doctor or dental assistant will give you full instructions on how to properly recover from surgery.
